Enrollment Forms
Online Enrollment is required for all families. Please complete the following online form for new family registration. Please ensure to include all authorized pick-up and Emergency Contacts for your family.

The following forms will be sent via our Procare app after your online Enrollment Form has been completed and accepted:
Registration Forms are required by the State of New Hampshire Child Care Licensing Unit for ALL registered children and MUST be on file prior to your child's first day of attendance.
Health Assessment Forms are required for ALL children. Any generic form provided by your child's health care provider is sufficient. A current Health Assessment AND Immunization History MUST be on file prior to your child's first day of attendance. An Immunization Exemption Form may be completed in lieu of an Immunization Report.
Automated Payment Forms are required for all families.
Scholarship Agreement Forms are required for all families who are approved by the State of New Hampshire for the Child Care Tuition Assistance Scholarship Program. One form will be provided via Procare per family. To apply for State Scholarship, visit the link below.
